A scapholunate dissociation (SLD), which progresses through several stages, finally ends as a perilunate dislocation.
A person who experiences pain in the back of the wrist or the thumb side of the wrist (following a fall) should be examined for scapho-lunate dissociation. The most common cause of scapholunate dissociation is fall on the out-stretched hand. The trauma cases scapholunate ligament ruptures, resulting in separation of lunate from scaphoid. It also causes the scaphoid to rotate dorsally. A gap of more than 3mm is pathognomonic for scapholunate dissociation. Rupture of the scapholunate ligament is the first event in any sequence of perilunar ligament ruptures. It is the most common pattern of wrist instability.
